PROOF OF CITIZENSHIP

• Certified birth certificate from the county or state where you were born
• Naturalization certificate
• Certificate of citizenship
• Previous U.S. passport book or card

PROOF OF IDENTITY

• Valid driver’s license
• Certificate of Naturalization with identifiable photo
• Military or military dependent identification card
• Government employee identification card
• Valid passport book or card

• IF you are printing your DS-11 at home please ensure that the form prints on two separate pages, not front and back.

[ ] A clear photocopy of front and back of ID

[ ] One 2X2 passport photo
