Documents Requirements

DS-11 Form

• A completed passport application (form DS-11) for first time and renewal applicants.
• The DS-11 form can be found HERE.
• If you are printing your DS-11 at home please ensure that the form prints on two separate pages, not front and back.

ORIGINAL PROOF OF CITIZENSHIP

• Certified U.S. birth certificate
• Previously issued, valid, U.S. passport book or card
• Report of Birth Abroad
• Certificate of Naturalization
• Certificate of Citizenship

EVIDENCE OF PARENTAL RELATIONSHIP

Parents must submit evidence of relationship to the minor applicant. To establish the relationship between the parents or legal guardian and the minor, one of the following must be submitted with the application.

Proof of relationship for a minor is not established through a previous passport.

• Certified U.S. or foreign birth certificate with parents’ names (with a translation if in a language other than English)
• Report of Birth Abroad with parents’ names
• Certified adoption decree with adopting parents’ names
• Certified court order establishing custody or guardianship

EVIDENCE OF IDENTITY OF PARENTS

A photocopy of each identification document to be presented by a parent or guardian must be submitted with the minor’s passport application. If a parent is applying for multiple minors at the same time s/he must include a photocopy of identification with each minor’s application.

• Valid Driver’s License
• Valid US Military ID
• Valid U.S. Government Employee ID
• Valid U.S. or Foreign passport (with recognizable photo)
• Naturalization/ Citizenship certificate
• Permanent Resident Card
• B2 Visa/ Border Crossing Card

Note: If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053 “Statement of Consent." You must submit the completed form with the child's passport application. The parent that cannot go with the child must:

• Sign and date Form DS-3053 in the presence of a certified notary public, and
• Submit a photocopy of the front and backside of the ID that they present to the notary public with Form DS-3053.
