All children, regardless of age, including newborns and infants, must have their own passport.

To execute the application of a minor child under 16, BOTH parents must appear with the minor applying for a passport. Both parents, along with the child must be present.

If only one parent is able to appear in person, please submit a notarized form DS-3053, Statement of Consent, for the absent parent. Please make sure to include a front and back copy of the absent parent's identification along with the form.

DS-11 Form

  • A completed passport application (form DS-11) for first time and renewal applicants.
  • The DS-11 form can be found HERE.
  • If you are printing your DS-11 at home please ensure that the form prints on two separate pages, not front and back.

ORIGINAL PROOF OF CITIZENSHIP

  • Certified U.S. birth certificate
  • Previously issued, valid, U.S. passport book or card
  • Report of Birth Abroad
  • Certificate of Naturalization
  • Certificate of Citizenship

EVIDENCE OF PARENTAL RELATIONSHIP

Parents must submit evidence of relationship to the minor applicant. To establish the relationship between the parents or legal guardian and the minor, one of the following must be submitted with the application.

Proof of relationship for a minor is not established through a previous passport.

  • Certified U.S. or foreign birth certificate with parents’ names (with a translation if in a language other than English)
  • Report of Birth Abroad with parents’ names
  • Certified adoption decree with adopting parents’ names
  • Certified court order establishing custody or guardianship

EVIDENCE OF IDENTITY OF PARENTS

A photocopy of each identification document to be presented by a parent or guardian must be submitted with the minor’s passport application. If a parent is applying for multiple minors at the same time s/he must include a photocopy of identification with each minor’s application.

  • Valid Driver’s License
  • Valid US Military ID
  • Valid U.S. Government Employee ID
  • Valid U.S. or Foreign passport (with recognizable photo)
  • Naturalization/ Citizenship certificate
  • Permanent Resident Card
  • B2 Visa/ Border Crossing Card

1. PASSPORT APPLICATION FEE

  • Must be paid with check or money order.
  • Make check or money order payable to "U.S. Department of State"

2. EXECUTION FEE - $25.00

  • Can be paid with check or money order.
  • Make check or money order out to "UNM Global Education Office"
